• Sara King ARNP and Dr. Evans Komen ARNP offer psychiatric evaluations, diagnostic clarification, medication management, and psychotherapy.

• Medications are electronically prescribed, including controlled substances.EARLY REFILLS OF CONTROLLED SUBSTANCES WILL NOT BE AUTHORIZED.

• Jennifer Shaw LISW, Tasha Lowman LISW, and Renee Barbu LISW offer counseling services for children, adolescents, and adults. Renee offers marriage, family, and group counseling.

• Telepsychiatry to your home

  • For telepsychiatry services you are required to have a credit/debit card on file which will be charged at the time of service.

Fees For Service

We accept most major commercial insurance carriers and Medicare.  We do not accept Iowa Medicaid or the managed care organizations including Amerigroup, Molina, or Iowa Total Care. Please call to see if your insurance is one of the covered carriers. You can also contact your insurance carrier to verify that Cedar Circle Mental Health & Wellness is an accepted provider.

Standard Fee Schedule, this will be billed to your insurance carrier and your co-pay will be due at the time of service.

Psychiatric evaluation and medication management:

• Psychiatric Evaluation 60-90 minutes: $375 

• Established patient follow-up 30 minutes: $275 

• Established patient follow-up 45 minutes: $300 

• Established patient follow-up 60 minutes: $350 

Therapy Fees

• Initial intake 45-60 minutes: $225

• 60 minute therapy session: $175

• 45 minute therapy session: $150
