Cancels & No Show Policy

We require a minimum of 24 hours notice when canceling or rescheduling an appointment in order to avoid a late cancellation fee. Please contact the office at 319.252.8718 to cancel or reschedule appointments. 

Missed, canceled, or rescheduled appointments with less than 24 hours notice with Sara King and Evans Komen will incur a $100 charge and for therapy will incur a $50 charge, which are not covered by insurance and will be charged to your credit card on file.

• If you are more than 10 minutes late to a scheduled appointment, your appointment may be canceled and you may be charged.

Dismissal Policy

You have the right to terminate care at any time and for any reason. We reserve the right to terminate treatment under the following conditions: 

• If we suspect prescription medication abuse or undisclosed substance abuse. 

• If a prescription drug contract is violated at any time during your treatment.

• If we believe that our services are no longer beneficial for you. 

• If we believe that you would be better served by a higher level of care or by a different provider.

• If there is a pattern of frequent cancellations/no-shows.

• If you cancel or miss an appointment and do not reschedule within 30 days of the missed appointment. 

• If we deem interpersonal dynamics are not conducive to a therapeutic relationship.

• If you or a family member are hostile or aggressive towards providers, support staff, or other clients.
